Liver Injury: Pirfenidone may cause liver damage, which can be serious or even life-threatening. Your healthcare provider will monitor your liver function by checking liver enzyme levels before starting treatment and regularly thereafter—monthly for the first six months, then every three months, or as needed. If you experience severe nausea, vomiting, pain in the right side of your abdomen, extreme tiredness, loss of appetite, or yellowing of the skin or eyes, seek medical attention immediately.
Sunlight Sensitivity: This medication can increase your sensitivity to sunlight and UV light, heightening the risk of sunburn even on cloudy days or during winter. To protect yourself, apply sunscreen with an SPF of 50 or higher and wear long-sleeved clothing when outdoors. Limiting sun exposure and avoiding tanning lamps are also recommended, particularly if you are taking other medications that heighten sun sensitivity.
Severe Skin Reactions: Although very rare, serious and potentially life-threatening skin reactions such as Stevens-Johnson syndrome (SJS), toxic epidermal necrolysis (TEN), or drug reaction with eosinophilia and systemic symptoms (DRESS) may occur. If you notice skin changes such as rashes, blisters, redness, or peeling, or experience symptoms like fever and swollen lymph nodes, discontinue use and seek immediate medical care. If diagnosed with one of these conditions, you should cease using pirfenidone permanently.
Stomach-related Side Effects: Commonly, pirfenidone can cause stomach issues such as nausea, stomach pain, diarrhea, and indigestion, particularly in the first three months of treatment. These effects typically subside over time. However, consult your healthcare provider if they persist or worsen.
