Selegiline Warnings

High Blood Pressure: Using selegiline may lead to increased blood pressure, particularly if dosages exceed 10 mg for tablets or capsules per day, or 2.5 mg for orally disintegrating tablets (ODT). Consuming tyramine-rich foods or exceeding recommended doses heightens this risk. Always adhere to prescribed dosages.
Serotonin Syndrome: Combining selegiline with other drugs that increase serotonin levels, such as antidepressants, may lead to serotonin syndrome, a serious condition. Symptoms include rapid heart rate, sweating, muscle stiffness, fever, and confusion. Inform your healthcare provider about all medications you are taking to prevent this risk and seek immediate medical attention if symptoms occur.
Extreme Sleepiness and Sudden Sleep Episodes: Selegiline, which elevates dopamine levels, may cause sudden sleep episodes, even without prior drowsiness. The risk increases if combined with sedatives or medications that cause sleepiness, especially in individuals over 65.
Low Blood Pressure and Fall Risk: Selegiline may lower blood pressure, particularly upon standing, leading to dizziness and potential falls. Rise slowly from seated or lying positions and consult your provider if you experience lightheadedness or falls. This is especially relevant for older adults or those on blood pressure medications.
Uncontrollable Body Movements: The medication might cause or worsen involuntary movements, such as eye-twitching or lip-smacking. Contact your healthcare provider if these occur, as dosage adjustments may be necessary.
Hallucinations and Behavioral Changes: Some users have experienced hallucinations, confusion, or behavioral changes. This medication is typically not recommended for those with a history of mental health issues. Report any such changes to your provider immediately.
Intense Urges: Selegiline may lead to compulsive behaviors, such as gambling or excessive eating. Notify your healthcare provider if you or those around you notice such urges.
Phenylketonuria (PKU) Caution – Zelapar ODTs Only: Inform your healthcare team if you have PKU, as this condition affects phenylalanine metabolism.
Contraindications: Avoid using this medication if you are currently taking, have recently taken, or plan to take within the next 14 days other monoamine oxidase inhibitors (MAOIs), linezolid, opioids, St. John's wort, cyclobenzaprine, dextromethorphan, or medications containing dextromethorphan, as these combinations can pose serious health risks. Always discuss your entire medication regimen with your healthcare provider before starting selegiline.
Selegiline Side Effects

Selegiline Interactions
Interactions with high risk of serious adverse effects and should be avoided:
- Antidepressants (including bupropion, Maprotiline, Mirtazapine)
- Other MAO inhibitors (isocarboxazid, Linezolid, Metaxalone, methylene blue, moclobemide, phenelzine, procarbazine, Rasagiline, safinamide, Tranylcypromine)
- Drugs for attention deficit disorder (such as Atomoxetine, Methylphenidate)
- Fluoxetine (ensure a five-week gap before starting selegiline)
Interactions with moderate risk that may require dose adjustment, closer monitoring, or timing changes:
- Appetite suppressants (such as Diethylpropion)
- Certain herbal products (such as ephedra/ma huang)
- Cold medications/nasal decongestants (such as phenylephrine, phenylpropanolamine, pseudoephedrine)
- Stimulants (such as amphetamines, ephedrine)
- Certain 'triptans' used to treat migraine headaches (such as Rizatriptan, Sumatriptan, Zolmitriptan)
- Certain opioid medications (such as Meperidine, methadone, pentazocine, propoxyphene, Tramadol, tapentadol)
Interactions with low risk that usually do not require a change in therapy:
- Tryptophan, tyramine supplements
